Consent Form

I consent to participating in the promotion "SFAP" (the "Advertisement") taking place in Sydney, NSW during August, September and October 2008, and duties will then fall into Autumn Carnival in 2009 at Royal Randwick racecourse.

I further consent to Australian Jockey Club Ltd, Royal Randwick, Maverick Communications, (collectively referred to as "the Companies") photographing using his/her image and recording his/her voice and performance and distributing the same as part of the Advertisement and as part of an associated online / web-based advertising campaign in Australia.

I hereby release the Companies and each of them from and against all claims, liabilities, demands, actions, loss, damage, costs and expenses brought by, on behalf of, or through arising directly or indirectly out of or related to the Advertisement and associated online advertising campaign including all future claims.

I acknowledge that the contents of this form and all information, discussions and documents related to the Advertisement and associated online advertising campaign are confidential to the Companies and I agree that I will not disclose any such confidential information to any third party without the prior written consent of the Companies.

I have read this form and the supporting script and understand their content. By signing this form, I acknowledge and agree that I will be legally bound by its contents.
